Cal-IF Member Feedback

This form is for SDP participants and their supporters who would like Cal-IF’s help resolving a concern, disagreement, or issue with a Cal-IF member. The goal is to provide a supportive process to better understand what happened and, when possible, help identify a path forward.

Once you submit the form, Cal-IF will confirm that the Independent Facilitator is a Cal-IF member and review the information you provided. Depending on the situation, Cal-IF may contact you for additional information, speak with the Independent Facilitator, and help clarify expectations or resolve the concern.

Cal-IF will handle the information as confidentially as possible and will let you know when the review is complete and whether there are recommended next steps.

This process is limited to concerns involving Cal-IF members. Cal-IF cannot resolve concerns involving Regional Centers, FMS agencies, or other vendors that are not Cal-IF members.
